What are your hours of operation?
Alarys Home Health is available 24-hours a day, 7 days a week. A R.N. is always on call and will be able to assist you with your medical questions. Office hours are from 8am to 5pm.

How do I make a referral?
Our referral form is located here; please be sure to include Doctors Orders, insurance information/authorization, demographics and medical history. Please also be sure to include your contact information so we can contact you if needed.

You will need:
- Current Licensure in area of Expertise
- Current CPR card
- Fingerprint Clearance card (may apply at Alarys or Police Dept; Fees involved)
- Immunization records or DR’s documentation of ability to work
- Current Tuberculosis or Chest X-Ray
- Proof of Auto insurance
- 2 forms of I.D. (Drivers license, Social Security card, Passport, Alien Resident card etc.)

Can I work with patients close to my home?
yes, an advantage of home health is that you can have patients close to your home. Our staff strives to place you in homes that are convenient to your locale. You have the ability to create your own schedule and take as many or as few cases as you desire.
Does Alarys Home Health offer insurance?
Alarys offers full benefit packages for those who maintain a 32 hour work week for a consecutive four weeks. Dental plans are also available.

What should I look for in a Home Health Agency?
A patient should always look for an agency that is accredited, has licenses, certifications and high quality care. Ask questions, and verify the agency you decide to utilize is an active member of the Better Business Bureau (BBB).

How do I obtain home health services for myself or loved one?
Private pay is available for our services if insurance is not a feasible option. A representative will come to the home and complete a complimentary safety evaluation and discuss what services are needed and desired. A price will be determined based on the services requested.
Medicare:
What are the requirements to utilize my Medicare benefit?
- You must be under the care of a doctor, and you must be getting services under a plan of care established and reviewed regularly by a doctor.
- You must need one or more of the following:
- Intermittent Skilled Nursing
- Physical therapy
- Speech-Language pathology
- Continued Occupational therapy
- You must be home-bound, and a doctor must certify that you’re home-bound. Being home-bound means:
- Leaving you’re home isn’t recommended because of your condition
- Your condition keeps you from leaving home without help (such as a wheelchair or walker, needing special transportation, or getting help from someone)
- Leaving your home takes a considerable and taxing effort
Please note leaving home for medical treatment (I.E. DR visits) or short, infrequent absences for non-medical reasons (such as attending religious services) are not a violation of Medicare’s Policies. Home healthcare is still available to those whom attend adult day care as well. Please also note that Home Heath services may include medical social services, part-time or intermittent home health aide services, medical supplies for use at home, durable medical equipment, and/or injectable osteoporosis drugs.
